Who We Support

Rutledge Center supports adults with intellectual and developmental disabilities aged 19 and older.

HOW TO APPLY FOR SERVICES

Adults aged 19 and older with developmental disabilities are eligible to apply for Medicaid Waiver services through the Department of Behavioral Health and Developmental Disabilities (DBHDD). Eligibility is initially handled through DBHDD and all individuals interested in services will need to be placed on the state planning list which is administered through DBHDD. For more information about this process please visit here.

This web-based portal allows individuals and families to submit applications for IDD services and track their application through the process in real time. Individuals and their families can view their application determination online and submit secure messages regarding their application.

Items that will be needed to submit your online application include:

  • Official Birth Certificate
  • Medicaid/Medicare Card
  • Psychological Evaluation
  • Social Security Card
  • Developmental Evaluation
  • School IEP Report
  • School Transcript
  • Vocational/Support Employment Records
  • Permanent Residence Card
  • Social Security Benefit Letter
  • Other Medical or Diagnostic Reports

Once you are ready to submit your application, click here to see a video with instructions and additional information for submitting your application.

Rutledge Center does not participate in the Waiver application and approval process.  Once an individual is determined to be eligible and most in need by the state office of DBHDD, the individual may choose to participate in Waiver Services via the Rutledge Center.

SCHOOL TRANSITION PROGRAM

There is often a gap in services for students between high school and the attainment of adult services. Rutledge Center can help fill this gap and provide individuals with disabilities services that will enable them to have increased independence, socialization, life skills, job development and other necessary resources that will help prepare them for the next phase in their life. Contact the Rutledge Center Director, Ann Newton, to learn more about the transition and how we can help you along the way.
